Your care continues

Even when you return home, Axiom Health Solutions continues to look after you. For the next six months your Personal Care Manager and your Consulting Surgeon will continue to monitor your progress and help transition primary care back to your family physician. They will check in regularly to be sure everything is progressing well. Because they have been a key part of your healthcare team, they can tell if your recovery is progressing properly.

Even after the six-month follow up is finished, Axiom Health Solutions is not. We stay with you on the path to better health. You have peace of mind with our Annual Follow Up and Update program, staying in touch to monitor your progress — for as long as you want — you decide when it stops.
